The Licensed Practical Nurse works under a registered nurse’s supervision to coordinate care delivery.

Duties/Responsibilities:

• Provides nursing care per the established plan of care.
• Provides ongoing physical and psychosocial assessment.
• Educates patients and families on disease management, medication administration, and preventative care.
• Communicates significant clinical changes or findings to the registered nurse or physician.
• Maintains accurate and up-to-date medical records and documentation timely.
• Collaborates with other healthcare professionals to ensure holistic patient care.
• Performs scheduled weekend visits and assists the on-call registered nurse when indicated.

Education:
Graduate from a licensed practical nursing program required.

Experience:
Two years nursing experience, home care preferred.

Licensure/Certification:
Valid Kentucky Board of Nursing (KBN) License required. Basic Life Support (BLS) Certification within 90 days of hire required. Valid Kentucky driver’s license required.
